A pesar de que las tasas de incidencia están disminuyendo, sigue siendo un problema de salud pública. La información y el conocimiento sobre la epidemiología de este tumor es abundante. Aquí se presenta una revisión bibliográfica actualizada sobre los factores de riesgo mejor documentados y aceptados por la comunidad científica: infección por Helicobacter pylori, dieta, susceptibilidad genética y nivel socioeconómico. Además, se analizan formas de enfrentar esta patología, por medio de la prevención (consumo de frutas y verduras) y la detección temprana con Rayos X y marcadores biológicos (pepsinógenos séricos), en poblaciones o grupos identificados como de alto riesgo. Se hace hincapié en las interrogantes y retos que plantean los nuevos conocimientos.</p></abstract>
<abstract abstract-type="short" xml:lang="en"><p>Gastric cancer is one of the tumors with higher worldwide mortality. Although the incidence rate has been declining, it is still a major public health problem still. Epidemiological information and knowledge about this cancer is abundant. This is an up to date review about commonly accepted risk factors: Helicobacter pylori infection, diet, genetic predisposition and socioeconomic status. We also stress the new questions and challenges that recent discoveries about this pathology entail. The ways to tackle this cancer are discussed using X rays and biological markers such as serum pepsinogens in high risk groups or populations.</p></abstract>

Metodos: se realizó un estudio prospectivo en 12 pacientes, los cuales fueron sometidos a cirugía de cadera, osteosíntesis con pin placa en fractura transtocantérica de fémur, y cuyo estado general fuera clasificado como ASA III o IV. Se le aplicó bloqueo femoral tres en uno y fémoro cutáneo lateral ipsilateral. Se utilizó buvicaína o.25%, lidocaína 2%, llevada a 40 ml de volumen. Resultados: la muestra incluyó siete varones y cinco mujeres. La edad media fue de 68.3 años, con límites entre 37 y 93 años. Los pacientes mostraron asociados diagnósticos de hipertensión arterial, diabetes mellitus, asma, cardiopatía isquémica, disrritmias, encefalopatía. Conclusiones: La estabilidad hemodinámica observada fue excelente, así como la analgesia, durante todo procedimiento y recuperación. La analgesia suplementaria fue mínima y el porcentaje de fallo fue menor del 9%. La gran estabilidad hemodinámica, la técnica sencilla y la alta tasa de efectividad hacen del bloqueo femoral tres en uno, una excelente opción para pacientes ASA III y IV con fractura transtocantérica de cadera, que serán sometidos a cirugía.</p></abstract>
<abstract abstract-type="short" xml:lang="en"><p>Twelve three in one femoral blocks were done for osteosynthesis with pin plate in transtrochanteric hip fracture in patients ASA III and ASA IV. It was used bupivacain 0,25% and lidocain 2% to a 40 ml volume. Observed hemodynamic stability was excellent, thus analgesic during the procedure and recover. Supplementary analgesic was minimal and the fail rate was less than 9%. Great hemodynamic stability, the simplicity of the procedure and the high efectiveness rate, made of the three in one femoral block an excellent choice for patients ASA III and ASA IV with transtrochanteric hip fracture that will need surgery.</p></abstract>

Sus complicaciones tienen gran impacto en la autonomía de estas personas. Una de sus complicaciones microvasculares es la retinopatía diabética, que si no se diagnostica tempranamente y se le interviene lleva a la pérdida de la visión. Su tratamiento es eficaz, logrando disminuir hasta en dos tercios el riesgo de ceguera, si se siguen dos recomendaciones: hacer un diagnóstico temprano y proporcionando tratamiento con foto-coagulación cuando sea indicado. En Costa Rica no se conoce la prevalencia de esta complicación y tampoco cual es la alternativa de solución de su atención en nuestro sistema de salud. Objetivo: Describir cómo se ha enfrentado la búsqueda y atención de la retinopatía diabética en un área de salud del Área Metropolitana, considerando los diferentes niveles de atención; así como analizar la prevalencia de la retinopatía diabética, después de implementado un tamizaje para su detección. Método: Se revisaron los expedientes de 572 diabéticos tipo 2 registrados al 31 de diciembre del 2000 en 6 EBAIS del Área de Salud 3 de Desamparados, anotándose el resultado del fondo de ojo que no tuviera más de dos años de antigüedad. A los pacientes que carecían de este examen se les contactó y se les realizó el fondo de ojo por personal experimentado. Resultados: El 93% de los diabéticos no tenían fondo de ojo en los últimos 2 años independientemente del nivel de atención donde llevara su control. Más aún, alrededor del 90 % no tenía documentado un fondo de ojo en ningún momento en el expediente. El tamizaje realizado logró aumentar la cobertura de fondo de ojo a un 60 %. Se obtuvieron datos de fondo de ojo de 345 pacientes (60.3%). De éstos, 27.5% presentaron algún tipo de retinopatía diabética; 16.5% no proliferativa, 8.7% no proliferativa severa y un 2.3% con retinopatía proliferativa. Fueron diagnosticados con el tamizaje 30 pacientes con retinopatía no proliferativa severa que requerían tratamiento con foto-coagulación de inmediato. Tres de esos paciente con edema macular requerían foto-coagulación urgente y se encontró dificultad para referirlos al III nivel de su área de atracción por no haber citas prontas. Conclusión: El presente estudio en comunidad demostró que no se está detectando la retinopatía diabética por cuanto no se hace fondo de ojo a los pacientes y hay dificultad para su intervención en el III nivel.</p></abstract>

Es una de las enfermedades psiquiátricas más debilitantes con un alto costo económico, social, familiar y personal. En Costa Rica, la esquizofrenia constituye la primera causa de internamiento entre los desórdenes psiquiátricos en el Hospital Nacional Psiquiátrico. El presente estudio analiza el diagnóstico preliminar de los primeros 120 pacientes incluidos en el proyecto sobre la Genética de la Esquizofrenia. Al comparar el diagnóstico del entrevistador, el diagnóstico final por consenso y el diagnóstico del último egreso hospitalario se encontraron diferencias en alrededor del 40% de los sujetos en el estudio. Se encontró un relativo sub-registro de la sintomatología afectiva en los diagnósticos de egreso con el consecuente sobre-diagnostico de los trastornos esquizofrénicos en los hospitales nacionales. El trastorno esquizoafectivo fue uno de los diagnósticos con menor concordancia. Las diferencias diagnósticas encontradas pueden ser parcialmente explicadas por el uso de pautas diagnósticas diferentes y/o una mayor rigurosidad diagnóstica utilizada en investigación que no es necesaria en la clínica. Sin embargo, la concurrencia de diferentes síntomas en las enfermedades psiquiátricas severas y las limitaciones impuestas por la categorización diagnóstica, obliga al psiquiatra a encasillar al paciente en una categoría que puede no representar completamente la enfermedad de ese paciente. El contar con marcadores biológicos en un futuro podría servir para clasificar estos síndromes en entidades diagnósticas más precisas, a explicar la fisiopatología de estas enfermedades y mejorar el tratamiento.</p></abstract>
<abstract abstract-type="short" xml:lang="en"><p>Approximately 1% of the world population is affected with schizophrenia. It is one of the most debilitating psychiatric disorders with a high economic, social, familial and personal cost. In Costa Rica, it is the first cause of hospitalization among psychiatric disorders at the National Psychiatric Hospital. This study analyses the diagnosis of the first 120 subjects included in the Genetic Study of Schizophrenia. Approximately 40% of the cases were discordant for the diagnosis when comparing the diagnosis of the interviewer, the final consensus diagnosis and the last discharg diagnosis. The last hospital discharge diagnosis showed a relative sub-registry of affective symptoms and an over-diagnosis of schizophrenic disorders. The diagnosis of schizoaffective disorders was the least concordant. The differences found could be partly explained by the use of different diagnostic criteria and/or by a more rigorous application of these criteria in research. Nonetheless, the concurrence of different symptoms in the severe mental disorders and the limitations imposed by strict diagnostic criteria, might give a diagnosis to a patient which doesn’t completely represent his/her illness. The future availability of biological markers might help to classify these disorders in more precise clinical entities, to better understand the physiopathology and improve treatment.</p></abstract>

En este trabajo se reporta el caso de una paciente de origen noruego, que presentó episodios diarreicos de larga evolución y tratamientos inadecuados después del diagnóstico, y el tratamiento correcto para esta parasitosis resolvió el cuadro, además, es el segundo caso reportado de esta parasitosis en nuestro país y el primero en el Hospital San Rafael de Alajuela, pretende alertar a los profesionales en Medicina y Microbiología, para que presten mayor atención ante la posible existencia de un número mayor de casos.</p></abstract>
<abstract abstract-type="short" xml:lang="en"><p>We are reporting a case of a Norwegian patient who presented large as recurrent diarrheic episodes. A diagnosis of cyclosporidiosis was stablish. This is the second case of cyclosporidiosis reported in Costa Rica and the first diagnose are de they patients were give the adecuated tratamient Hospital San Rafael de Alajuela. This is a call for doctors and microbiologist to be prepared for more possible cases.</p></abstract>

En este estudio se da cuenta de las características clínicas y el manejo de 15 pacientes del Hospital Calderón Guardia, diagnosticados entre 1987 y 2001. Todos los casos fueron de localización torácica y en doce de ellos la enfermedad afectó solamente los ganglios del mediastino, por lo que la radiografía de tórax fue el método de detección . Los síntomas más frecuentes fueron tos y disnea. La mediastinoscopía fue el método más utilizado para obtener la biopsia. El diagnóstico histológico se hizo por el hallazgo de granulomas no necrotizantes, infiltrado linfocitario y la presencia de células gigantes, sin que se aislara ningún germen. Catorce pacientes recibieron tratamiento sintomático y observación y solo uno presentó afección pulmonar y sintomatología respiratoria y recibió esteroides. El paciente con enfermedad pulmonar sintomática continúa con esteroides y los pacientes observados, por un promedio de 7 años, evolucionaron a resolución espontánea sin producirse recidivas.</p></abstract>
<abstract abstract-type="short" xml:lang="en"><p>Sarcoidosis is an infrequent disease in Costa Rica. In 1988 the Hospital San Juan de Dios reported six cases. This is a clinical study of 15 patients diagnosed at the Hospital Calderón Guardia from 1987 to 2001. All the patients presented with thoracic disease and in 12 of them it was localized to the mediastinal lymph nodes. A chest X-ray detected all the cases; cough and dispnea were the most frequent symptoms and mediastinoscopy was the most useful method to obtain a biopsy. The histologic diagnosis was made in the presence of non-necrotizing granulomas, giant cells and a lymphocytic infiltrate. No microorganisms were isolated. Fourteen patients received symptomatic treatment and observation, and one with pulmonary disease and respiratory symptoms received also steroids. All the patients were observed, for an average of 7 years. They presented spontaneous resolution without remissions.</p></abstract>

Hay pocos casos reportados de asociación entre la ingesta de propiltiouracilo, la aparición de anticuerpos anticitoplasmáticos antineutrófilos y vasculitis ANCA positiva. Este es probablemente uno de ellos.</p></abstract>
<abstract abstract-type="short" xml:lang="en"><p>We report a case of 43-year-old woman who developed two complications associated with the use of propylthiouracil: vasculitis and agranulocytosis. Vasculitis associated with antithyroid therapy is a rare well-documented clinical entity with a pathogenic mechanism not clear yet. There a few published reports of an association between treatment with propylthiouracil, and the occurrence of ANCA positivity and ANCA-associated vasculitis. This is probably one more.</p></abstract>
