Se estima que de un 60 a 84% de los pacientes con cáncer desarrollan metástasis óseas. De estos un 70% experimentan síndrome s dolorosos de difícil manejo, de los cuales un 50% muere sin un adecuado alivio del dolor con una pobre calidad de vida. Por tanto, es necesario contar con medicamentos accesibles y efectivos para el manejo de esta patología. El objetivo de este artículo es revisar la fisiopatología del dolor en hueso y describir los medicamentos que se utilizan con más frecuencia en el manejo de este tipo de dolor por cáncer. Además se presenta un algoritmo de 6 pasos que permite orientar al médico al momento de tomar una decisión terapéutica.
Sixty to 84% of cancer patients develop bone metas tases. Of this 79% have difficult to treat bone pain syndromes, of which 50% die without adequate pain control. It is necessary to have accessible and effective drugs in the market to improve the quality of life in these patients. The objective of this paper is to review the pathophysiology of bone pain and the drugs used today in the palliation of bone cancer pain. In addition a 6 step algorithmic approach is given to help the physicians engaged in the management of this condition.
Objetivo: Describir los motivos de consulta de los inmigrantes que asistieron a los servicios de salud del primer nivel de atención del distrito primero del cantón Central de Alajuela pertenecientes al área de salud de Alajuela Norte, durante 2002. Métodos: Se trata de un estudio descriptivo de corte transversal. Se recogieron las siguientes variables: edad, sexo, nacionalidad, sede de EBAIS donde que se consultó y motivo de la consulta. Resultados: Se registraron 1076 consultas de inmigrantes, correspondientes a 467 personas extranjeras. Del total de consultas, 853 (79.3%) se otorgaron a mujeres. El promedio por paciente fué de 2.3 consultas y la edad media, de 39.8 años. Respecto a la nacionalidad, 772 (71.7%) consultas se realizaron a pacientes nicaragüenses, 140 (13%) colombianos y 41 (3.8%) salvadoreños. Los tres diagnósticos o motivos de consulta que mayor demanda de servicios de salud registraron fueron: control de patología o exámenes de laboratorio, infecciones de vías respiratorias superiores e infecciones del tracto urinario. El EBAIS que atendió a más inmigrantes fue Cristo Rey con 241 consultas (22,4%). Conclusiones: Los pacientes inmigrantes que asisten a los servicios de salud del primer nivel de atención del Distrito Primero del Cantón Central de Alajuela pertenecientes al Área de Salud de Alajuela Norte, durante 2002, son jóvenes, predominantemente de sexo femenino.
Objective: To describe the reasons of consultation of immigrants who attended the health services at the first attention level at the First District of the Central Canton of Alajuela that belongs to "Area de Salud Alajuela Norte", during 2002. Methods: This is a cross-sectional descriptive study. The following variables were collected: age, gender, nationality, consult place reason of consultation. Results: A total of 467 foreigners consulted 1076 times. Among all consults, 853 (79.3%) were made by women and 223 (20.7%) by men. The average of the consults was 2.3 consults per patient. The median age was 39.8 years old. Regarding nationality, 772 (71.7%) consults were made by nicaraguan patients, 140 (13%) by colombian patients and 41 (3.8%) by salvadorans. The main reasons for consultation were testing control of an existing disease process or for laboratory testing, upper respiratory infections and urinary tract infections. The place where most immigrants consulted was the EBAIS form Cristo Rey with 241 (22.4%) consultations. Conclusion: The immigrant patients who attend first level health services of attention are young people, principally females and with diseases similar to the Costa Rican population from the same district.
Justificación y Objetivos: los enterococos son cocos Gram positivos y las especies de importancia médica son Enterococcus faecalis y E. faecium como productores de la mayoría de las infecciones en el ser humano y E.casseliflavus, E. gallinarum, E. durans, E.hirae, E.raffinosus, y E.avium raramente aislados de muestras clínicas pero que pueden poseer genes extracromosómicos que los hacen intrínsicamente resistentes a los glucopéptidos, los que podrían ser transferidos a E. faecalis y E. faecium. La colonización por estas bacterias es frecuente en pacientes gravemente enfermos ingresados en unidades de cuidado intensivo(VCI) y es un factor predisponente para septicemia. Los objetivos.de este trabajo son la identificación de pacientes colonizados por enterococos vancomicina resistentes (EVR) en medios hospitalarios nacionales, el análisis de la sensibilidad a antibióticos de las bacterias aisladas, el estudio de los factores de riesgo para adquiridas y de los aislamientos con alta resistencia a la vancomicina, con el fin de identificar determinantes genéticos de resistencia. Pacientes y métodos: estudio prospectivo, que se desarrolló de mayo a agosto del año 2001. Se incluyeron 106 pacientes internados en las unidades de cuidado intensivo del Hospital México, Hospital San Juan de Dios y Servicio de emergencias médicas de ese último nosocomio, a los que se les tomaron hisopados rectales, los que fueron cultivados para identificar la presencia de EVR. Se determinaron las concentraciones mínimas inhibitorias (CIM) para vancomicina, y se realizó el estudio genético de las cepas con CIMs más altas. Resultados: la tasa de colonización del tracto gastrointestinal de los pacientes fue de 52%. Empleando las pruebas de chi cuadrado y de regresión logística, se identificaron factores que intervienen en la colonización por estas bacterias, siendo el servicio de procedencia antes del ingreso a las respectivas VCI, los días de estancia en VCI y el uso previo de cefalosporinas de tercera generación los principales. El 29.6% de los aislamientos de enterococos resistentes a vancomicina tenían CIM >512 ug/ml, todos poseían el gen van A y correspondieron a E.gallinarum, E.faecium, E casseliflavus y E.hirae Todos los aislamientos de Enterococcus faecalis mostraron CIM ₤16ug/ml a la vancomicina. Conclusiones: se demostró 52% de prevalencia de colonización en pacientes con EVR, superior a lo encontrado en otros estudios publicados en la literatura médica. Se establece la importancia de conocer estos hallazgos para el manejo clínico y epidemiológico de las infecciones asociadas a estas bacterias. Pocos estudios en la literatura mundial han logrado identificar este tipo de determinantes genéticos de resistencia (vanA) en enterococos no patogénicos. Se discute la relevancia de estos datos.
Justification and Objectives: Enterococcus are Gram positive bacteria involved in community and nosocomial infections. There has been an increased interest in these bacteria because of their resistance to multiple antimicrobial drugs and their transposon-mediated resistance to glycopeptides. Colonization of patients with enterococci is a risk factor for septicemia. Our main objectives were identification of patients colonized by vancomycin resistant enterococci (VRE) in two tertiary care hospitals, analysis of the antibiotic sensitivity to vancomycin of the isolated bacteria, study of the risk factors to acquire these bacteria and study of the isolations with high resistance profile to the vancomycin with the purpose of identifying genetic determinants of resistance, were the objectives of this study. Patients and methods: this is a prospective, and experimental study conducted from may to august 2001. We studied 106 patients hospitalized in the Intensive Care Units (ICU) of the Hospital México and Hospital San Juan de Dios and also the Medical Emergency Ward of the latter. Rectal swaps were taken in order to isolate VRE and to study the minimal inhibitory concentrations of vancomycin. Strains isolates with higher MIC's were studied for genetic determinants Results: VRE colonization rates in gastrointestinal tract were 52%. Using the chi square test and logistic regression some factors were identified for gastrointestinal tract colonization, being service of origin before admission to ICU, days of stay in ICU and previous use of third generation cephalosporins the main ones. VRE with MIC µ512 mg/ml were demonstraded in 29.6% of the isolates, and in all of them (E.gallinarum, E.,faecium, E. casseliflavus, and E.hirae) the van A gen was found. All E. faecalis isolates showed vancomycin MIC£16 mg/ml. Conclusions: We found a high colonization rate of the gastrointestinal tract, higher than the reported prevalence in other studies in the medicalliterature. Few studies in world wide literature have identified the van A gene in non-pathogenic enterococci. The relevance of these findings is discussed.
El tratamiento de la hipercolesterolemia primaria (HCP) se basa en una dieta reducida en grasa saturada y en colesterol, con la cual se puede disminuir entre 10 y 20% del colesterol total (CT). La proteína de soya tiene el potencial de reducir el CT, cuando sustituye parcial o totalmente a la proteína animal. El presente estudio reporta los resultados del efecto de una dieta baja en grasa saturada, colesterol y con proteína de soya, sobre el colesterolligado a lipoproteína de baja densidad (LDLC) en niños costarricenses con HCP. En un ensayo clínico paralelo de asignación aleatoria, 20 niños, entre los 7 y 14 años de edad fueron asignados a uno de los dos tratamientos dietéticos según los criterios de inclusión -grupo experimental: dieta etapa 1 recomendada por el NCEP, más 25 gramos de proteína de soya diarios y, -grupo control: dieta etapa 1 NCEP, sin proteína de soya. Después de un período de homogenización de diez días, se inició la intervención por ocho semanas. Tanto los niños del grupo experimental como los del grupo control, redujeron el colesterol total y el LDL-C (p< 0,05). Sin embargo, a pesar que no hubo diferencias significativas entre los grupos, se notó una mayor reducción en el LDL-C del grupo experimental (10,5 % vs. 3,9%). Los que consumieron más de 20 gramos de proteína de soya diaria, tuvieron mayor reducción en el LDL-C y un efecto negativo menor en el HDL-C.
Primary hypercholesterolemia treatment (PHT) is based on a reduced diet in saturated fat and cholesterol, which can diminish between 10 and 20% of serum cholesterol (CT). Soybean protein has the potential to reduce CT, when it substitutes partial or totally animal protein. The present study shows the results of the effect of a low saturated fat, cholesterol and soybean protein diet on LDL-C in Costa Rican children with primary hypercholesterolemia. Twenty children, between the ages of 7 and 14, were randomly assigned to one of the two dietary treatments at home. The experimental group received: Stage 1 NCEP diet, it consisted of 25 more grams of soybean protein and the control group received: Stage 1 NCEP diet without soybean protein. After a homogenization period of ten days, the intervention began for eight weeks. Both groups reduced their total cholesterol and their LDL-C (p< 0,05). However, the experimental group showed a greater reduction of their LDL-C (10,5 % vs. 3,9 %1), than the control, but tbis difference was not significant. Those who consumed more than 20 grams of soybean protein had a bigger reduction of their LDL-C and a smaller negative effect of their HDL-C. Soy protein diet did not have significant effect on LDL-C reduction, We can not conclude that there is an advantage of using soy protein over traditional treatment of primary hypercholesterolemia.
El osteosarcoma telangiectásico es una lesión maligna rara que constituye aproximadamente un 2,5 % de todos los osteosarcomas primarios. Ha sido asociado con un curso más agresivo que el osteosarcoma convencional en series clínicas grandes. Por su comportamiento clínico, aspectos radiológicos e histopatológico es muy semejante a otros tumores benignos como el quiste óseo aneurismático. Presentamos el caso de un paciente masculino de 24 años de edad sano, con dolor crónico en su rodilla izquierda como único síntoma. Fue diagnosticado con esta rara forma de osteosarcoma y amputado. Recibió un régimen de 6 meses de quimioterapia, después del cual le fueron encontradas metástasis pulmonares y en la pared torácica. Es importante reconocer esta variante de osteosarcoma y diferenciada de lesiones benignas porque solamente la resección, como tratamiento, conlleva un pronóstico reservado, mientras que si se adiciona quimioterapia, la expectativa de vida mejora. Además de esto, es importante recordar esta entidad en el diagnóstico diferencial del miembro doloroso en gente joven y sana.
Telangiectatic osteosarcoma is a rare malignant lesion which constitutes about 2,5 % of all primary osteosarcomas. It has a more aggressive course than conventional osteosarcoma, as seen in large clinical series. By its clinical behavior, radiological and histopathological aspects, it is very similar to other benign tumors, such as aneurismatic bony cyst. We present here the case of a 24 year old male, with chronic pain in his left knee as only symptom. He was diagnosed with this rare form of osteosarcoma, amputated and then received a six month chemoteraphy regimen, after that, lung and thoracic wall metastasis were found. Therefore, recognition of this variant of osteosarcoma and its differentiation from benign lesions is important because resection alone carries poor prognosis, whereas if chemotherapy is added to the treatment plan the life expectancy improves. This condition should be kept in mind in the differential diagnosis of limb pain among young and healthy people.
La intoxicación por opiáceos se presenta casi siempre como consecuencia de una sobredosis accidental de un derivado de éstos, usualmente la heroína por inyección. Otras formas de intoxicación, a las que no somos ajenos en nuestro medio, son los llamados correos de drogas, "body packers" o mulas, utilizados en el tráfico internacional de transporte de drogas. En estos casos la droga es generalmente tragada, envuelta en condones, que pueden contener heroína o cocaína. Su ruptura puede conducir a toxicidad severa y letal. Los antecedente de un viaje reciente en avión en un paciente con cuadro clínico de intoxicación, pueden justificar la indicación de una radiografía de abdomen como método diagnóstico e incluso la cirugía para limpieza intestinal como parte de su abordaje terapéutico.
Most of opioid related deaths for overdose are accidental and are primarily due to intravenous heroin use. Other forms of poisoning may occur in the so called body packers ("mulas"), who are used in international drug transportation. These patients generally swallow small quantities of wrapped cocaine or heroin in an attempt to smuggle the drugs across international borders. Their breakage can lead to severe and lethal toxicity. The history of a recent a flight in a patient with a clinical syndrome of poisoning, should justify the indication of abdominal x-rays for diagnosis and also surgery for "intestinal cleaning" as part of his therapeutic approach.
