Debido a la morbilidad asociada a esta, es vital su reconocimiento temprano y el manejo adecuado, con la finalidad de reducir la incidencia de complicaciones asociadas y mejorar la calidad de vida de quienes la padecen.</p></abstract>
<abstract abstract-type="short" xml:lang="en"><p>Atrial fibrillation is the most frequent arrhythmia seen in clinical practice.It occurs most commonly on patients with hypertensive, isquemic and valvular heart disease as well as in patients without cardiac disease principally in elderly patients. Due to the morbidity rate associated with it (atrial fibrillation), early diagnosis and adequate treatment is necessary in order to reduce any associated complication and to improve the health related quality of life of patients who suffer from it.</p></abstract>

Es por ello que se describen las condiciones técnicas y profesionales mínimas para la adecuada administración de una sedación.</p></abstract>

Sin embargo, no facilitan información precisa de lo que sucede en las diferentes áreas de salud del país, ni del desempeño de los programas preventivos. El presente estudio pretende determinar el estado nutricional de la población menor de 5 años que asistió a la consulta de crecimiento y desarrollo en el distrito de San Rafael de Alajuela, durante el periodo 2002, y comparar la situación nutricional de la población en estudio, con los resultados de obtenidos en la última Encuesta Nacional de Nutrición, de 1996. Métodos: Se trabajó con la población de niños(as) menores de 5 años que asistió a la consulta de crecimiento y desarrollo del primer nivel de atención en salud. A partir de la muestra obtenida, se caracterizó a la población, según los indicadores antropométricos peso/edad, talla! edad y peso/talla. Resultados: Se revisó un total de 275 expedientes. La mediana de edad fue de 18 meses y la edad más frecuente de consulta fue de 12 meses. Según el indicador peso/edad, 33 (12%) niños(as) presentaron algún grado de desnutrición y 9 (3.3%), sobrepeso. El número de niños(as) con problemas de desnutrición y sobrepeso fue semejante en ambos sexos; igualmente, la cantidad de pacientes con problemas de desnutrición y sobrepeso en el grupo de niños(as) menores de 1 año, fue semejante al de 1 a 4 años. Conclusión: Al comparar los resultados obtenidos con la última encuesta nacional de nutrición, se determinó que los (as) niños(as) que asisten a la consulta de crecimiento y desarrollo, presentaron un mejor estado nutricional que el de la población nacional.</p></abstract>
<abstract abstract-type="short" xml:lang="en"><p>Nutritional surveys carried out in Costa Rica provide an overview of the general nutritional status of our population. However, they don't provide accurate information as to what happens in different health areas in the country or data about the performance of prevention programs. The present study, therefore, is aimed at determining the nutritional status of the population under 5 years of age who visited the primary care centers in the district of San Rafael, province of Alajuela in 2002, and to comparing the nutritional status of the target population with the results from the last national nutritional survey from 1996. The target population consisted of children under 5 years old who visited the primary care centers. Based on the sampIe, a population characterization was carried out according to anthropometrical indicators of weight/age, size/age, and weight/size. A total of 275 records were analyzed. The mean age was 18 months and the most frequent age was 12 months. According to the weight/age indicator, 33 (12%) of the children showed certain degree of malnutrition and 9 (3.3%) had overweight. The percentage of children suffering from malnutrition or overweight was similar for both genders and ages. After comparing results fram the last national nutritional survey, it was determined that children who visited the primary care centers showed a better nutritional status than the national population.</p></abstract>

No se ha documentado la importancia de fortalecer la inocuidad de alimentos, lo cual produce eventos que comprometen la salud pública cada año, por lo que fue importante estudiar el presente brote. En este estudio se analiza el brote diarreico de un centro penitenciario de San José, en 2003, procurando determinar el probable alimento contaminado. Materiales y métodos: El presente estudio corresponde a una cohorte retrospectiva que identifica exposición a los alimentos preparados dentro del Centro, en las 72 horas previas al pico epidémico. La descripción de la cohorte y el cálculo del (RR) se realizó posteriormente para determinar la posible&amp;#9;asociación causal. Las muestras de heces fueron procesadas en el laboratorio de referencia del INCIENSA. Resultados: La totalidad de los individuos pertenecían al sexo masculino, todos privados de libertad, con un promedio de edad de 31.1años (IC95%=28.9-33.4). La sintomatología más frecuente fue la diarrea (92.3%) Y dolor abdominal tipo cólico (84.6%). El resultado de laboratorio aisló como agente etiológico el Clostridium perfringens enterotoxigénico (CPE). Al analizar los alimentos suministrados, pico epidémico y período de incubación, se tiene que el salchichón (RR=1.1;IC95%0.5-2.7) y la preparación con carne molida (RR=2.1,IC95%:0.6-7.4) mostraron ser los únicos alimentos capaces de transmitir el CPE. Discusión: El brote del Centro evidencia que la sintomatología descrita corresponde con lo documentado para la intoxicación por CPE y que, tomando en cuenta las condiciones necesarias para la transmisión, solo el salchichón y la preparación con carne molida cuentan con esta plausibilidad biológica. Al considerar el periodo de incubación, los valores de riesgo relativo calculados y la potencia estadística, se concluye que el alimento probablemente contaminado fue la preparación realizada con carne molida, por lo que se deben de reforzar las medidas de control sanitario.</p></abstract>
<abstract abstract-type="short" xml:lang="en"><p>Introduction: The sicknesses transmitted by foods appear annually in Costa Rica. They are usually studied until the etiologic agent is identified and but not until the source of contamination is determined. In Costa Rica there are no records of the importance of improving the inocuence of foods by the events that compromise the public health each year therefore the importance to study the present outbreak. In the present study a diarrheic outbreak in a penitentiary center of San José in the 2003, trying to determine the pos sible contaminated food. Materials and methods: The present retrospective cohort study identify as exposition the foods prepared in the center within the 72 hours previous to the epidemic peak. The description of the cohort and the calculus of the RR was made posteriously to determine the possible casual association. The samples of feces were processed in the laboratory of reference INCIENSA. Results: The totality of the individuals were sex male, all inmates, average age 31.19 years (CI95%=28.92-33.46) The most frequent symptoms are diarrhea (92.3%) and abdominal colic pain (84.6%). Laboratory results isolated as etiologic agent the Clostridium perfringens entorotoxigenic (CPE). Analyzing foods supplied, epidemic peak and incubation period, we have sausage(RR=1.19,CI95%0.520-2.710) and the preparation of grind meat(RR=2.18,CI95%:0.638-7.47), show to be the only food to transmit the CPE. Discussion: The outbreak in the center evidence that the symptoms described correspond with documentation for intoxication by CPE and taking in consideration only necessary conditions for the transmission only with sausage and preparation of grind meat, counting with this biologic plausibility. Considering incubation period, calculated relative risks and the statistics power, we conclude that the probable contaminated food was the preparation made with grind meat, so that sanitary measures must be reinforced.</p></abstract>

En ocasiones, el temor de los padres produce la conocida &quot;fobia febril&quot;, que conlleva al uso de los servicios de emergencias hospitalarios y a tratamientos inadecuados. Objetivo: Conocer la percepción, el conocimiento y el manejo que dan a la fiebre los padres o encargados de familia, de aquellos niños hospitalizados en el servicio de Infectología del Hospital Nacional de Niños &quot;Dr. Carlos Sáenz Herrera&quot; de Costa Rica. Materiales y métodos: Se realizó un estudio prospectivo descriptivo, donde se entrevistan a los padres o encargados de niños hospitalizados en el Servicio de SI-HNÑ, entre enero y abril de 2001. A los padres ó encargados que cumplían con los criterios de inclusión y exclusión, se les explicó el propósito del estudio y la dinámica del cuestionario. Resultados: Se recolectaron un total de 100 cuestionarios, donde se documentó que e161 % de los entrevistados consideran necesario el uso del termómetro para hacer el diagnóstico de fiebre, el 43% define como fiebre toda temperatura &gt; a 38 &amp;#8734;C. La complicación más temida en el 75% de los consultados es la convulsión. El 71 % cuenta con algún tipo de tratamiento, en su hogar, para el manejo de la fiebre. Conclusión: Este estudio demuestra que muchos de los entrevistados no disponen de conocimiento adecuado acerca de la fiebre, sus implicaciones y manejo, lo que hace necesario promover información basada en la evidencia, que ayude a los padres, para que sepan cómo actuar correctamente ante la presencia de este importante y común síntoma.</p></abstract>
<abstract abstract-type="short" xml:lang="en"><p>Introduction: Fever is one on the main reasons for pediatric consultation, and sometimes paren tal concem to it may produce &quot;fever phobia&quot;. This may lead to inadequate use of hospital emergency consultations and inappropriate treatments. Objective: To elucidate the degree of perception, knowledge and treatment given by parents to the febrile children admitted to the SI- HNN. Materials and methods: We performed a prospective descriptive study including parents or legal guardians of those children admitted to the Infectious Diseases Division of the Hospital Nacional de Niños de Costa Rica, between January and April of 2001. To those who met inclusion and exclusion criteria, a questionnaire was given prior explanation of the study dynamics. Results: A total of 100 questionnaires were collected. Sixty one percent of those who were interviewed considered as essential the use of a thermometer to make the diagnosis of fever, and 43% defined fever as any temperature above 38°C. Development of seizures was the most feared complication in 75% of parents. At home, seventy one percent of them have some type of treatment for fever. Conclusions: This study demonstrates that many parents who were interviewed do not have an adequate knowledge of fever, its implications and treatment. This raises the necessity of promoting evidence-based information that may help parents to deal with this common and important symptom.</p></abstract>

No existe ningún caso previamente reportado en el país. Método: Reportamos 2 niñas con sospecha clínica de padecer de síndrome de Angelman. Ambas son pacientes del Departamento de Neurología del Hospital Nacional de Niños y la información fue adquirida de sus expedientes clínicos. Resultados: Las 2 pacientes presentan las 4 características clínicas: retardo severo del desarrollo, deterioro profundo del habla, ataxia y una disposición feliz y sociable. Además, manifiestan, como convulsiones asociadas a espigas típicas y ondas lentas en el EEG, y atracción por el agua. Conclusión: El diagnóstico es difícil, debido a que otros desórdenes clínicos pueden imitar las características del síndrome de Angelman. Sin embargo, a edades tempranas, el fenotipo conductual de disposición feliz e hiperexcitabilidad es la manifestación más importante y decisiva en el diagnóstico diferencial de pacientes con retardo psicomotor y en el desarrollo del lenguaje.</p></abstract>
<abstract abstract-type="short" xml:lang="en"><p>Justification and objective: Angelman syndrome is a severe neurological disorder. No other case has been reported in our country until now. Methodos: We report two children with the clinical suspicion of Angelman syndrome. They were treated at the Childrens National Hospital Neurology Department. The information was taken from their medical records. Resultd: the two patients present the four cardinal clinical features, including severe developmental delay, profound speech impairment, ataxia and a happy, sociable disposition. In addition, the patients displayed other characteristics: seizures associated with a typical spike and slow wave activity on EEG an love for water. Conclusion: The clinical diagnosis is difficult because other disorders can mimic the features of Angelman Syndrome. Nonetheless, at an early age, the behavioral phenotype of happy disposition and hyperexcitability is the most important manifestation and appears to be decisive in the differential diagnosis of patients with psychomotor and language delay.</p></abstract>

Es el primer adulto con enfermedad de Chagas aguda tratado en Costa Rica con benznidazol. Se citan los síntomas encontrados, así como los aspectos epidemiológicos relacionados con su presentación. Se enfatizan el tratamiento y la evolución del caso. Se hace un llamado de atención a los clínicos, ya que este caso no fue diagnosticado apropiadamente desde un inicio, lo que evidencia el desconocimiento que existe de la enfermedad de Chagas como motivo de consulta.</p></abstract>
<abstract abstract-type="short" xml:lang="en"><p>An acute case of Chagas &quot;disease with Romañas&quot; sign, in a 48 years old man, is described. Treatment with Benznidazol was succesfull. This is the first acute case in an adult treated with this drug in Costa Rica. Reference is made to the epidemiological aspects involved in this case and to the mechanism of infection. This person became infected when he was trying to kiU a nymph of T. dimidiata with a coffe stick and received a jet of urine directly in the right eye. The case was not diagnosed by the clinicians while the patient was at the hospital indicating the poor knowledge of practitioners about the characteristics of the disease in our country.</p></abstract>
