La frecuencia de dicha arritmia aumenta con la edad, presentándose en un 1.5% de los 50 a 59 años a 10% de los 80 a 89 años. La fibrilación atrial no valvular incrementa el riesgo de sufrir un evento cerebrovascular isquémico cardioembólico en 5 veces y causa el 15% de todos los accidentes cerebrovasculares isquémicos en Estados Unidos de América. El manejo de la fibrilación atrial se enfoca, principalmente, en la prevención de los fenómenos tromboembólicos y en el control de la frecuencia y ritmo cardiaco. La anticoagulación, cuando está indicada, ha demostrado ser la principal herramienta en la prevención de dichos eventos. Sin embargo, aunque las complicaciones hemorrágicas son más frecuentes, en esta población, y aumentan con la edad, sobrepasa por mucho, el beneficio al riesgo. El control de la frecuencia cardiaca ha demostrado ser igual o mejor que el control del ritmo en cuanto a prevención de eventos cerebrovasculares y mortalidad en estos pacientes. <abstract abstract-type="short" xml:lang="en"><p>Atrial fibrillation is the most prevalent arrhythmia in the elderly. Its frequency increases with age, being 1.5% from 50 to 59 years old and 10% from 80 to 89 years old. Non valvular atrial fibrillation increases 5 fold the risk of suffering an stroke and causes 15% of strokes in the USA. Atrial fibrillation management focuses in the prevention of thromboembolic phenomena and heart rate and rhythm control. Anticoagulation, when indicated, has demonstrated to be the main tool in the prevention of these events. Nevertheless, although bleeding complications are more frequent in this population and increase with age, anticoagulation benefits are greater than the risks. Heart rate control is better than rhythm control regarding cerebrovascular accidents and mortality. Age by itself is not a contraindication to offer optimal therapy. Functional, mental and social status, must be taken into account as well as physiological aspects of aging when it comes to prescribing medications. If symptoms persist in spite of adequate treatment, invasive strategies have demonstrated to be of benefit, however studies in elderly population are lacking.</p></abstract>

El consumo de suplementos de vitaminas antioxidantes ha ido en aumento, bajo la premisa de que estos podrían ayudar a prevenir o curar enfermedades crónicas. Sin embargo, a pesar de la gran cantidad de investigaciones que se han realizado en este campo, no se ha logrado esclarecer realmente el papel que la suplementación con estos micronutrientes pueda estar ejerciendo en la patología de esas enfermedades. El Instituto Nacional de Salud de los Estados Unidos establece, que no existe, suficiente evidencia científica para promover o desestimular, el uso de suplementos multivitamínicos para la prevención de las enfermedades crónicas. Los datos emergentes cuestionan los beneficios reales que los suplementos antioxidantes puedan tener en los sujetos con un adecuado estado nutricional, y sugiere que su consumo podría no ser seguro en comparación con las vitaminas y minerales contenidos naturalmente, en los alimentos. Los meta-análisis realizados, con estudios randomizados no han logrado demostrar efectos benéficos de la suplementación con multivitaminas sobre la mortalidad por enfermedades crónicas, y más bien, algunos análisis han sugerido que los &amp;#946; -catorenos, la vitamina A y la vitamina E podrían aumentar el riesgo de muerte.</p></abstract>
<abstract abstract-type="short" xml:lang="en"><p>Observational studies have consistently demonstrated a positive association between high fruits and vegetables intake and a reduced risk of heart disease, cancer, cerebrovascular disease, and other chronic diseases. Under the premise that vitamin and mineral supplements can have the same effects of those naturally found in food, many people are using them to prevent or treat chronic diseases. But in spite of intensive research, the benefitial effect of antioxidant supplements is still not clear. The US National Institute of Health states that there is not enough evidence to recommend or discourage the use of multivitamin supplements to prevent chronic diseases. Growing scientific data questions the benefitial value of supplementing with antioxidant vitamins, in well nourished population and raises the possibility that they may not be safe compared with vitamin consumption within a healthy diet. Meta-analyses of randomized trials have failed to demonstrate that supplementations with antioxidant vitamins can have benefitial effects on mortality. Even more, some analyses suggested that &amp;#946;-carotenes, vitamin A or vitamin E can increase the risk of death.</p></abstract>

Métodos: fuentes de información fueron los boletines epidemiológicos del Ministerio de Salud del 2001 al 2007 y el Instituto Nacional de Estadísticas y Censos. Se calcularon tasas de incidencia por sexo, grupos de edad, provincia y cantón. Las tasas del último trienio se compararon con una investigación que describe la situación epidemiológica de 1973 a 1975, para identificar cambios epidemiológicos de la enfermedad. La revisión bibliográfica permitió describir las formas clínicas y la distribución geográfica de los vectores que la ocasionan. Resultados y conclusión: la incidencia aumentó de 10,5 del 2001 a 40,7 x100.000 habitantes en el 2007. Los menores de 19 años fueron los más afectados. Talamanca presentó la mayor incidencia cantonal. El agente etiológico más frecuente de la leishmaniosis cutánea es la Leishmania (Viannia) panamensis, aunque también existe la L. (V.) braziliensis. Los mosquitos trasmisores fueron Lutzomyia ylephiletor y L. trapidoi. Los reservorios del parásito son los perezosos: Bradypus griseus y Choloepus hoffmanni y un roedor Heteromys desmarestianus. Existe también la leishmaniosis cutánea atípica, producida por Leishmania infantum (= Leishmania chagasi), productora responsable de un caso de leishmaniosis visceral. L.longipalpis, esta asociada a la transmisión de estas dos últimas formas de leishmaniosis. Debe entrenarse al personal de salud, en la detección de esta enfermedad, y educar a la población.</p></abstract>
<abstract abstract-type="short" xml:lang="en"><p>Justification and aim: We conducted an investigation to study the epidemiological behavior of the disease, its related vectors and the species that cause leishmania. Methods: As sources of information we used epidemiological bulletins from the Costa Rican Ministry of Health covering the years 2001 to 2007 and also those from the National Institute of Statistics and Census. The rates of incidence of the disease were calculated according to sex, age group, and distribution in different provinces and counties. These rates of incidence for the last 30 years were compared to an investigation that described the epidemiology of the disease from 1973 to 1975, in order to identify epidemiological changes of the disease. Having studied the bibliography of information compiled concerning this disease, we were able to describe the clinical forms and the geographic distribution of its vectors. Results: The incidence increased from 10,5 per 100.000 inhabitants found in 2001 to 40,7 per 100.000 inhabitants found in 2007. Young people of less than 19 years of age were found to be the most affected. The greatest rate of incidence for the country was found in Talamanca. The main etiological agent of cutaneous leishmaniasis is Leishmania (Viannia) panamensis, although L. (V.) braziliensis also exists. The vectors agents were Lutzomyia ylephiletor and L. trapidoi mosquitoes. The natural carriers of these parasites found in the wild are Bradypus griseus and Choloepus hoffmanni sloths and Heteromys desmarestianus rodents. There are also cases identified of atypical cutaneous leshmaniasis, produced by Leishmania infantum (= Leishmania chagasi), also responsible of a case of visceral leishmaniasis. L. longipalpis has been found to transmit these two forms of leishmaniasis. Conclusion: We found that it is important to train health workers on detecting leshmaniasis and educate the general population about this disease.</p></abstract>

El personal hospitalario puede ser fuente potencial de infección para estos pacientes, ya que normalmente actúa como portador de gérmenes, que eventualmente podrían ser transmitidos a los pacientes. Se describe, en esta investigación, el aislamiento de hongos levaduriformes a partir de las manos y la cavidad oral, en un grupo de 77 trabajadores del Hospital San Juan de Dios, en servicios donde se han reportado más casos de infecciones por este tipo de hongos. Métodos: Se realizó un hisopado de cavidad oral y manos de cada participante y se cultivaron placas con agar glucosado de Sabouraud (AGS) y Mycosel. A los aislamientos de levaduras se les determinó la capacidad de crecimiento a 37°C, resistencia a la cicloheximida, producción de tubo germinativo, fosfolipasas y proteinasas y se determinó la sensibilidad in vitro al fluconazol por medio de método de microdilución en placa. Resultados: El 72,7% de los participantes resultaron positivos para el aislamiento de levaduras, la especie aislada con mayor frecuencia fue Candida parapsilosis, seguida de C. albicans y C. famata. La mayor positividad se obtuvo en el servicio de Cirugía 3 (83.3%), seguido de la UCI (71.4%) y Neonatología (58%). Conclusión: Estos resultados instan a mejorar las acciones preventivas en el manejo de los pacientes, a ser más estrictos en las normas de higiene de manos y promover, en otros centros hospitalarios, la realización de este tipo de estudios, para disminuir los brotes nosocomiales por transmisión horizontal.</p></abstract>
<abstract abstract-type="short" xml:lang="en"><p>Justification and aim: Critically ill patients are more susceptible than the general population to opportunistic fungal infections. Health workers could be a potential infectious focus to these patients. Thus in this investigation we report the isolation of yeast from the hands and oral cavity in a group of 77 employees of the San Juan de Dios Hospital from Services where infecctionsdue to these fungi had previously been reported. Methods: Samples from oral cavity and both hands were taken from 77 individuals. Each sample was platted into Sabouraud´s dextrose agar and Mycosel agar. Each yeast isolate was analyzed for growth capacity at 37°C, cycloheximide resistance, germ tube formation, phospholipase and proteinase production. Further, in vitro susceptibility testing of each isolate to fluconazol was performed using a microdilution method. Results: A 72.7% yeast positivity was found in all samples taken. Candida parapsilosis was the most frequent isolate, followed by C. albicans and C. famata. The ward with the greatest positivity was Surgery 3 (83.3%), followed by Intensive Care Unit (71.4%) and Neonatology (58%). Conclusion: These findings suggest that strict aseptic handling of patients should be observed to avoid horizontal transmission of yeasts in these wards. Similar studies should be conduced in others hospitals.</p></abstract>

En el coprocultivo se identificó Shewanella algae. Casos de infección en humanos por esta bacteria son raros y se han encontrado, predominantemente localizadas, en piel, oídos y tejidos blandos con o sin bacteremia, en la mayoría de los cuales existe el antecedente de contacto con ambientes marinos. Shewanella algae es sensible a aminoglicósidos, carbapenemes, eritromicina y quinolonas, pero resistente a penicilinas. La sensibilidad a ampicilina y cefalosporinas es variable. Nuestro paciente fue tratado con cefalexina y tuvo una respuesta satisfactoria al mismo, con cese de los síntomas. De acuerdo con nuestro conocimiento este el primer caso documentado, en Costa Rica, de infección gastrointestinal por esta bacteria.</p></abstract>
<abstract abstract-type="short" xml:lang="en"><p>We report herein the case of a male patient, from Pavas, San Jose, with chronic diarrhea whose stool culture grew Shewanella algae. Infections caused by this micro-organism are infrequent and most commonly involve ears, skin and soft tissue, with or without bacteremia and are usually linked to ocean contact in warm climate countries. Shewanella algae is sensitive to aminoglycosides, carbapenems, erythromycin and quinolones, but resistant to penicillin. Sensitivity to amplicilin and cephalosporins is variable. Our patient was treated with cefalexin, with satisfactory results. To our knowledge, this is the first documented case in Costa Rica of gastrointestinal infection caused by this microorganism.</p></abstract>

Los padres consultan a los 2 días de vida pues le notan dificultad respiratoria, hipoactividad y rechazo a la leche materna. El paciente se interna y se aborda como una sepsis. Durante su estancia en el servicio de neonatología del Hospital Nacional de Niños asocia fallo respiratorio que amerita ventilación mecánica asistida por varios días en diferentes ocasiones, derrame pleural exudativo, convulsiones de origen hipóxico isquémico. Con reporte de hemocultivos positivos por Estreptococos pyogenes. El Estreptococos pyogenes o estreptococo &amp;#946;-hemol&amp;#957;tico del grupo A, fue un problema en los comienzos del siglo pasado, siendo frecuente en las infecciones puerperales y del reci&amp;#953;n nacido. En la actualidad es un germen sumamente raro en los procesos de sepsis neonatal.2 La gravedad de la enfermedad causada por este microorganismo en el periodo neonatal var&amp;#957;a desde una onfalitis crónica de bajo grado a una septicemia, una meningitis fulminante y la muerte.1 El presente artículo pretende hacer un resumen del paciente, con su evolución clínica, radiológica y además ejemplificar todas las complicaciones que tuvimos con este germen tan poco frecuente en la actualidad en sepsis neonatal.</p></abstract>
<abstract abstract-type="short" xml:lang="en"><p>We present herein the case of a newborn patient of appropriate gestational age weight ( 3700 grams), born by vaginal delivery, from a mother that had had 2 previous pregnancies (2 normal deliveries). During the immediate puerperium she had fever. The parents consulted at the age of 2 days, stating that they had noticed difficult breathing since his birth, hipoactivity and poor appetite. He was admitted to the hospital and underwent several studies searching for the origin and germ causing the sepsis. He developed respiratory failure and needed mechanical ventilation for several days on different occasions. He had exudative pleural effusion and hypoxic ischemic seizures. Later on, his blood cultures were positive for Streptococcus pyogenes. Streptococcus pyogenes or Streptococcus &amp;#946;- hemolytic group A infection, used to be a common entity in the beginning of the past century, it was commonly associated with puerperal infections and newborn infections, but its incidence declined and nowadays, is uncommon during the neonatal period. Its clinical manifestations could vary from chronic omphalitis up to a low grade septicemia or fulminant meningitis. Since its incidence has declined, at the present time; is an unusual infection of the neonatal period. Ampicillin and gentamicin are currently recommended as first-line antimicrobials, ampicillin replacing the previously recommended penicillin.</p></abstract>
