El objetivo del presente estudio fue determinar la prevalencia de diabetes tipo 2, intolerancia a la glucosa y diabetes provisional, como una forma de detección oportuna en la población adulta que se realizó por cualquier motivo, un examen de glicemia en ayunas en la Clínica de Salud de El Guarco de Cartago. Se revisaron los expedientes (excluyendo los de mujeres embarazadas) de las personas de 15 a 75 años de edad que acudieron durante el año 1998, a la Clínica de El Guarco a realizarse una glicemia en ayunas. Se tomaron datos demográficos, antropométricos y antecedentes personales de enfermedades crónicas. Se consideró, un valor de glicemia &lt;110 mg/dL como normal, entre 110-126 mg/dL como intolerancia a la glucosa (ITG), &gt; 0:126 mg/dL diabetes provisional (D M provisional). Los diabéticos diagnosticados fueron aquellos que en su expediente estuvieran como diabéticos ya conocidos. La prevalencia de DM fue de 4,2%; 1,3%, ITG y 0,2% diabetes provisional. El riesgo de ITG y de DM aumentó significativamente con la edad. Los obesos (IMC &gt; 30) tuvieron 3,7 (IC 95% 2,09-6,84) más riesgo de padecer DM y 3,24 (IC 95% 1,02-13,5) más riesgo de presentar ITG. La prevalencia de diabetes en esta comunidad fue más alta que la reportada en el país, la de ITG es más baja que en otros países. Ambas tienden a aumentar con la edad y el grado de obesidad.</p></abstract>
<abstract abstract-type="short" xml:lang="en"><p>Diabetes Mellitus Type 2 is one of the main public health problems worldwide, affecting to 5-7% of population. To determine the prevalence of Diabetes Mellitus (DM), Glucose Intolerance (GI) and Provisional Diabetes (PO), attended in the Health Center of El Guarco-Cartago. Medical files from all adults 15-75 year-old (excluding pregnant and diabetics) which glucose levels were evaluated after 12h of fasting in the Health Center of El Guarco-Cartago during 1998 were revised. Demographic and anthropometric data, individual history of chronic disease were registered. Glucose intolerance and provisional diabetes were defined as serum glucose level between 110-126 mg/dl and levels &gt;126 mg/dl, respectively. Serum glucose levels &lt;110 mg/dl were considered as normal. The prevalence of Diabetes Mellitus was estimated based on the diagnosis indicated in the medical file. He prevalence of DM, GI and PO was 4.2%, 1.3% and 0.2% respectively. The association analysis showed that the risk of GI and DM increase significantly with age. Obese adults (BMI30) have a risk 3.7 times higher (IC 95% 2.09-6.84) to develop DM than those adults with normal weight. Likewise, obese adults have a risk 3.2 times higher (IC 95% 1.02-13.5) to develop GI than those adults with a BMI between 18.5 - 24.99. The prevalence of Diabetes Mellitus Type 2 in the population under study was higher than the reported by Costa Rica. On the contrary the prevalence of GI was lower than the reported in other countries. This study showed that the prevalence of DM and GI trend to increase with both age and the obesity grade.</p></abstract>

Para ello se revisaron los expedientes (excluyendo los de mujeres embarazadas y de los diabéticos) de las personas de 15 a 75 años de edad que acudieron durante el año de 1998, a realizarse una glicemia en ayunas. Se tomaron datos demográficos, antropométricos y antecedentes familiares y personales de enfermedades crónicas, se utilizó un valor de &gt;110 mg/dL como glicemia elevada (GE). El 6,7% (n=46), presentó valores de GE. Se observó un gradiente biológico con la edad y el índice de masa corporal (IMC), es decir a mayor edad y mayor IMC, mayor riesgo de presentar GE. Los hipertensos (OR 6,38, IC95% 3,13-13,8) y los que ingirieron medicamentos (OR 3,32, IC95% 1,52-8-52) tuvieron mayor riesgo de presentar GE. El modelo de regresión logística, aportó que los antecedentes familiares de diabetes, ser hipertensos, la edad y IMC fueron los factores que se asociaron positivamente con la GE. Considerando que las personas con GE presentaron los mismos factores de riesgo descritos en personas con diabetes es necesaria la inclusión de éstas, en programas de promoción de la salud con el fin de prevenir la diabetes mellitus.</p></abstract>
<abstract abstract-type="short" xml:lang="en"><p>The proposite of this study is determine the risk factors associated to high serum glucose levels in adults attended in the Health Center of El Guarco-Cartago. Medical files from all adults 15-75 year-old (excluding pregnant and diabetics) which glucose levels were evaluated after 12h of fasting in the Health Center of El Guarco-Cartago during 1998 were revised. Demographic and anthropometrics data, family and individual history of chronic disease were registered. Serum glucose levels &gt; 110 mg/dl were considered as high. The prevalence of high serum glucose levels was 6.7%. The prevalence of serum glucose levels 110 mg/dl was significantly higher in females than in males. A positive linear relationship between age, body mass index (BMI) and glucose levels was evidenced. Adults with high blood pressure and those to take medicines showed a higher probability to present high serum glucose levels. The logistic regression analysis model indicated that the family history of Diabetes Mellitus (DM), age, BMI, and high blood pressure were factors positively associated to glucose levels 110 mg/dl. This study demonstrated that adults with family history of Diabetes Mellitus obesity present a higher risk to shown high serum glucose levels. Therefore, nutritional interventions and educational strategies are needed to promote the adoption of healthful lifestyle among to this population in order to preventt he DM development.</p></abstract>

Hasta diciembre del 2000 se habían completado 507 aloinjertos, por lo que este hospital se ha convertido en el principal centro de transplante de Centro América. En este período han fallecido 105 pacientes (20,7%), 66 de los cuales han sido sometidos a estudio postmortem. Los objetivos de este estudio fueron analizados la sobrevida, los principales hallazgos patológicos y las causas de muerte en dicha población autopsiada. Se revisaron las historias clínicas, los protocolos de autopsia y las lesiones histopatológicas de los 66 pacientes sometidos a transplante renal que fallecieron y que se les hizo autopsia entre 1973 y el 31 de diciembre del 2000 en el Hospital México. Los pacientes sobrevivieron por períodos variables desde menos de 1 mes (11 casos) hasta más de 1 año (33 casos), con un promedio de 33,4 meses. Las principales causas de muerte fueron: 1. Infecciones bacterianas y oportunistas: 25 casos (37,8%); 2. Rechazo del injerto: 17 casos (25%); 3. Complicaciones cardiovasculares: 11 casos (16,6% ) ; 4 . Trastornos hemodinámicos: 6 casos (9%). Las infecciones constituyen un peligro permanente para todo paciente inmunosuprimido. Sin embargo, desde la introducción de la ciclosporina (1987), la incidencia y severidad de las mismas ha disminuido notablemente. La disfunción crónica del transplante (rechazo crónico vascular; glomerulopatía crónica del transplante, etc.) se ha convertido en otro serio problema en el manejo a largo plazo de esta población. En efecto, sólo 4 de los últimos 31 pacientes fallecidos mantenían adecuada función renal al momento del deceso.</p></abstract>
<abstract abstract-type="short" xml:lang="en"><p>The first renal transplant was carried out in the Hospital México in 1973. Until December of 2000, 507 allografts had been completed, so that this hospital has become the main transplantation center in Central America. In this period 105 patients have died (20,7%), 66 of which have been subjected to postmortem study. The objectives of this study were to analyze the survival, the main pathological findings and the causes of death in this population. The clinical histories, the autopsy protocols and the histopathological lesions of the renal transplantation patients that have died and that have been autopsied between 1973 and December 31 the 2000 were reviewed. The patients survived for periods ranging from less than 1 month (11cases) to more than 1 year (33 cases), with an average of 33.4 months. The main causes of death were: 1. Bacterial and opportunistic infections: 25 cases (37.8%); 2. Reject of the implant: 17 cases (25%); 3.Cardiovascular complications: 11 cases (16.6%); 4. Hemodynamic dysfunction: 6 cases (9%). The infections constitute a permanent hazard for all inmunosupressed patients. However, since the introduction cyclosporin (1987), their incidence and severity has diminished notably. The chronic disfunction of the transplant (chronic vascular rejection; chronic transplant glomerulopathy, etc.) has become another serious problem in the long term management of this polulation. Indeed, only 4 of the last 31 patients who died maintained adequate renal function at the moment of death.</p></abstract>

Por esta razón, el objetivo de este estudio fue determinar la presencia de microorganismos en la zona subungal de un grupo muestreado de trabajadores de la salud de un hospital en Costa Rica. Se realizaron cultivos del material obtenido del área bajo las uñas de 46 trabajadores de la salud, el cual fue inoculado en agar sangre. Del 48% de esas personas se aisló al menos un microorganismo. Los agentes aislados más frecuentemente fueron Staphylococcus (12% S. aureus 24%, S. epidermidis y 22% S. warneri), que corresponde al 75% del total de aislamientos; también, se cultivaron tres cepas de Candida (C. tropicalis, C. guillermondi y C. parapsilopsis). Estos datos confirman la importancia de hacer un lavado concienzudo de las manos para como medida de prevención de las infecciones nosocomiales.</p></abstract>
<abstract abstract-type="short" xml:lang="en"><p>Hand cleansing remain the most effective and least expensive measure to prevent transmission of nosocomial infections; however, it is a practical that sometimes is neglected and permits the establishment of bacteria in fingernails, that could acts as a reservoir for those agents. For that reason, the aim of this study was the evaluation of the microflora of subungual region in health care workers from Costa Rican hospital. Culture obtained from 46 health workers from the material took under their subungual region; which was inoculated on blood agar. From 48% of these persons were isolated at least one microorganism. The most frequent agent cultured was Staphylococcus (12% S. aureus 24%, S. epidermidis and 22% S. warneri), that correspond to the 75% of the isolations; also, 3 strains of Candida (C. tropicalis, C. guillermondi and C. parapsilopsis) were cultivated. These data reaffirm the importance of conscientious hand cleansing to preventn osocomial infections.</p></abstract>

<self-uri xlink:href="http:/www.scielo.sa.cr/scielo.php?script=sci_arttext&amp;pid=S0253-29482003000100006&amp;lng=en&amp;nrm=iso&amp;tlng=en"></self-uri><self-uri xlink:href="http://www.scielo.sa.cr/scielo.php?script=sci_abstract&amp;pid=S0253-29482003000100006&amp;lng=en&amp;nrm=iso&amp;tlng=en"></self-uri><self-uri xlink:href="http://www.scielo.sa.cr/scielo.php?script=sci_pdf&amp;pid=S0253-29482003000100006&amp;lng=en&amp;nrm=iso"></self-uri><abstract abstract-type="short" xml:lang="es"><p>Se analizaron 420 muestras diarreicas de pacientes admitidos en el Hospital San Vito, Puntarenas; Costa Rica durante enero del 2001 a enero del 2002 y se determinó el porcentaje de resistencia de Shigella sp a 8 antibióticos de primera línea. El 65,1% de las cepas se aislaron durante los meses de octubre a diciembre de12001. Se aislaron 142 cepas de Shigella flexneri y 6 de Shigella sonnei. El 70% mostraron resistencia a TrimetoprinSulfametoxazole, tetraciclina y ampicilina, siendo todas las cepas sensibles a la gentamicina, cefotaxima y ciprofloxacina. El estudio mostró que la mayoría de las cepas aisladas presentaron una alta resistencia a los antibióticos comúnmente utilizados en el tratamiento de las enfermedades diarreicas. Por lo tanto, se recomienda un monitoreo continuo de los patrones de resistencia para un subsecuente tratamiento.</p></abstract>
<abstract abstract-type="short" xml:lang="en"><p>Among 420 diarrheic simples from patients from the San Vito Hospital, Puntarenas, Costa Rica, were essayed from 2001, January to 2002, January. The percentage of resistance of Shigella sp; to eight of the first line antibiotics, was determined. The 65.1 % of the strains were isolated between October and December of 2001. One hundred and forty two of the strains isolated were Shigella flexneri and six were Shigella sonnei. The 70% of the strains shown resistance to Trimethoprim / Sulfametoxazole, tetracycline and ampicillin. AII of the strains were susceptibles to gentamicin, cefotaxime and ciprofloxacin. The case studies determined that most of the strains isolated, have an higher resistant rate to the most used antibiotics in treatment against diarrheic illness. For hence, it is important the continual monitoring of the resistance patterns for a subsequentt reatment.</p></abstract>

Hasta 1970 la importancia médica de este agente se relegó a una micosis superficial en el pelo, caracterizada por la producción de unas masas blanquecinas, de donde deriva el nombre de &quot;Piedra blanca.&quot; Actualmente este agente se relaciona con infecciones sistémicas o localizadas en algunos órganos en pacientes con algún grado de inmunodeficiencia o con cuadros malignos bajo tratamiento, en los cuales se presentan leucopenias importantes. Otros factores predisponentes son el tratamiento con corticosteroides, la implantación de prótesis en válvulas cardiacas, la hemacromatosis y el SIDA. Se describe el caso de una niña de 3 meses de edad, con un absceso hepático del cual se tomó una biopsia por punción, que mostró lesiones caracterizadas por un infiltrado inflamatorio con predominio de células mononucleares, detritus celulares asociados con masas de micelio septado, artrosporado, blastosporas, algunas gemando y otras formando cadenas de pseudomicelio. Del cultivo de ese material se aisló Trichosporon beigelii. Histopatológicamente esta micosis se puede confundir con Candida, Aspergillus y Geotrichum. También, las colonias en cultivos jóvenes en agar sangre tienden a confundirse con Candida. En todo caso la identificación de micelio, pseudomicelio, blastosporas y artrosporas orienta al diagnóstico.</p></abstract>
<abstract abstract-type="short" xml:lang="en"><p>Trichosporon beigelii is a fungus isolated from soil and superficial water mainly from tropical countries. Also, it has been associated with human normal flora of ski n and mouth. Until 1970 its medical importance was regarded with &quot;White Piedra&quot; an infection that produces whiteness concretions attached to hair. Nowadays this agent is related with systemic infection with a high mortality, mainly in patients with immunodeficiency, associated HIV, malignant hematology disorders, treatments with cytotoxicity drugs or corticosteroids. In this paper we described the case of a three years old girl with a hepatic abscess. The histology of the biopsy material showed lesions characterized by inmflamatory infiltrates with predominance of mononuclear cells, cellular detritus and mycelial masses with blastospores, arthroblastospores, and pseudomycelia. A strain of Trichosporon beigelii was isolated from the biopsy tissue. At histopathologic analysis this micosis could be confounding with Candida, Aspergillus, or Geotrichum. Also, in blood agar the young colonies have a similar aspect of Candida. For those reasons, the observation of mycelia, blastospores, arthroplastospores, and pseudomicelia oriented the diagnosis.</p></abstract>
