Se evaluó la efectividad de la vacuna contra la hepatitis B (Engerix B) en 116 empleados del Hospital Nacional de Niños, San José, Costa Rica, de 120 que ingresaron voluntariamente al programa de vacunación en el período 1995-1996. La vacunación fue exitosa en el 99,13% de los individuos alcanzando éstos respuestas de seroprotección con niveles de anti HBs mayores a 10 mUI/ml. Se puede corroborar que la edad y el sexo tienen influencia directa sobre los niveles de seroconversión; en general son mejores respondedores los jóvenes y las mujeres.

The effectiveness of hepatitis B vaccine was evaluted in 120 employees at the National Children`s Hospital of San José, Costa Rica, who enter voluntarily in the vaccination programme during the 1995-1996 period. The vaccination results were found successful in 99,13% of the persons studied, showing seroprotection response levels of ant-HBs antibodies higher than 10 mUI/ml. It was found that age and sex had a direct influence on the seroconvertion levels reached, observing that young people and women were better responders.

Se determinó la presencia de anticuerpos (IgA) contra Chlamydia pneumoniae y Chlamydia trachomatis en el suero de 30 niños admitidos al Hospital Nacional de Niños por infecciones en el tracto respiratorio con edades entre los 7 días y los 28 meses. Se obtuvieron los siguientes resultados: 2/30 (6,6%) sueros fueron positivos para C. trachomatis y 8/30 sueros fueron positivos para C. pneumoniae (26.6%). Es la primera vez que se establece la presencia de C. pneumoniae en Costa Rica. Se discuten estos resultados en relación con los resultados de estudios por bacterias y virus, edad y diagnóstico de los pacientes.

El recién nacido cuyos genitales son ambiguos representa un reto para el pediatra y la familia. Una clara comprensión de las bases de la diferenciación del sexo y la consulta oportuna con un endocrinólogo pediatra es crítica para la evaluación y determinación del sexo de crianza del recién nacido congenitales ambiguos. El cariotipo sexual y una concentración de 17 - OHP pueden ser suficientes en la evaluación inicial de pseudohermafroditismo femenino debido a que la mayoría de los pacientes tendrán HSC virilizadora. Si se sospecha de pseudohermafroditismo masculino sobre la base de gónadas palpables, obtenemos de manera rutinaria, un cariotipo, concentraciones de esteroide suprarrenal basal y niveles de suero de testosterona estimulada por HGC y DHT, luego consideramos una prueba de tratamiento con testosterona. Los médicos que se preocupan por los niños con genitales ambiguos deben apreciar las necesidades culturales, religiosas y psicológicas de la familia y evitar determinar el sexo ante de que se llegue a un diagnóstico exacto.
