Las terapias que administran son estimulación antitaquicardia, cardioversión y desfibrilación y existen diversos métodos matemáticos mediante los cuales estos dispositivos pueden discriminar si una taquiarritmia es ventricular o supraventricular. Cuando hay errores en la discriminación, administran terapias en forma inadecuada con consecuencias negativas en la calidad de vida y la vida útil de la batería. Metodología: El objetivo de este estudio es conocer la prevalencia de terapias inadecuadas en los pacientes portadores de cardiodesfibriladores implantables que se han controlado en la clínica de arritmias de nuestro centro en el período junio-2002 a julio-2004. Se interrogó por telemetría a todos los pacientes y se estableció si la terapia administrada por el dispositivo era adecuada en base a los electrogramas almacenados. Resultados: Se registraron 99 eventos almacenados en 16 pacientes. El número total de visitas a la clínica de marcapasos fue 33 (2.05 visitas/paciente). De los 99 eventos registrados, se consideró 22 como inadecuados (22.22%); si se excluye del análisis a un paciente con displasia arritmogénica del ventrículo derecho que tuvo 64 eventos adecuados el mismo día y otro con 17 cardioversiones el mismo día debidas a fibrilación atrial aguda, se obtiene una prevalencia de terapias inadecuadas del 45.45%. Conclusión: La prevalencia de terapias inadecuadas es relativamente frecuente y la más común en esta serie fue la fibrilación atrial interpretada como fibrilación ventricular debidas a errores en la discriminación.</p></abstract>
<abstract abstract-type="short" xml:lang="en"><p>Introduction: Implantable cardiodesfibrillators are indicated in patients with history of cardiorespiratory arrest and/or ventricular tachycardia/ventricular fibrillation. Therapies delivered by these devices includes antitachycardia pacing, cardioversion and defibrillation and there are mathematical algorithms to discriminate if an arrhythmia diagnosed is of ventricular or supraventricular origin. Because of mistakes during interpretation these devices can deliver inadequate therapies with negative consequences in quality of life and battery’s lifespan. Methods: The main objective of this study is to evaluate prevalence of inadequate therapies in patients with implanted defibrillators who were follow-up in the arrhythmia department of our hospital since june-2002 to july-2004. All devices were interrogated by telemetry and arrhythmic events were evaluated by mean of stored electrograms, assessing if delivered therapy was adequate or not. Results: There were 99 arrhythmic events stored in 16 patients. The total number of visits to the arrhythmia department were 33 (2.05 visits/patient). From 99 stored events, 22 were considered as inadequate (22.22%); excluding one patient with arrhythmogenic right ventricular dysplasia who had 64 adequately treated episodes on the same day and another one with 17 cardioversions the same day due to acute atrial fibrillation, prevalence of inadequate therapies was 45.45%. Conclusion: Prevalence of inadequate therapies is relatively frequent and the most common in this serie is atrial fibrillation interpreted as ventricular fibrillation due to misdiagnosis.</p></abstract>

Se documenta mediante ecocardiografía transesofágica masa en tracto de salida del ventrículo derecho.</p></abstract>
<abstract abstract-type="short" xml:lang="en"><p>We present a clinical case about a young patient admitted with dysnea, chest pain and tricuspid regurgitation with right ventricle pressure over 70 mmHg and secondary right chambers enlargement in a transtoracic echocardiogram. An obstructive right ventricle outflow tract mass in the transoesophageal echocardiogram was documented.</p></abstract>
